AH! That might it explain it. I was using a custom mode - mostly. The few photos that were on card 2 were shot in 'A' mode instead of C1.

Dual Card slots
I'm trying to set up dual cards in my OM1mkii
On the spanner/wrench menu i chose Card Slot Settings and then Save Settings Dual Same.... -
